According to the Agency for Health Research and Quality (AHRQ), "Diagnostic error comprises a notable and costly fraction of all medical errors and has resulted in devastating consequences for patients, families, and health care professionals."

Failure to diagnosis is the single greatest contributor to paid medical malpractice claims. In some specialties, medical malpractice claims for errors in diagnosis eclipse medication errors by a factor of seven (Physicians Insurers Association of America, Risk Management Review - Neurology 2009).

A recent study of adverse events in ambulatory care illustrated that over one-third of these events were a reflection of breakdowns in diagnosis.
