Failure to diagnosis is the single greatest contributor to paid medical malpractice claims. In some specialties, medical malpractice claims for errors in diagnosis eclipse medication errors by a factor of seven (Physicians Insurers Association of America, Risk Management Review - Neurology 2009). A recent study of adverse events in ambulatory care illustrated that over one-third of these events were a reflection of breakdowns in diagnosis.
