Subject: Acute versus Gradual Switch From Pramipexole to Ropinirole (MovDis Congress 2000)

Acute versus gradual pramipexole to ropinirole switch

Forty-eight PD patients with motor fluctuations taking a mean of 2.5 mg/day pramipexole and concomitant levodopa were randomized to switch to ropinirole either overnight or gradually over 3 weeks. Dose equivalence used was 1 pramipexole=3 ropinirole. Three gradual and two acute patients withdrew prior to the 3-week evaluation. At week 3, 71% of the gradual group versus 46% of the acute group tolerated the switch with no or only mild difficulty (p=0.14), as determined by self-report, indicating the gradual switch "tended to be better tolerated." Seventy- one percent of the gradual group and 86% of the acute group reported deterioration in their motor status. Following the 3-week evaluation, ropinirole could be increased. At week 6, mean dose of ropinirole for all patients was 8.92 mg, suggesting a dose equivalence of 1 pramipexole=3.8 ropinirole.
